Vise Interchangeable Thumb Breaking?

I've had the ITs installed on every ball I've drilled over the last 5 years (probably a dozen balls or so) and in the last year I've had the bottom of the system (inside the ball, not on the slug) break 4 times.  Three of those times have been in the same ball, which was drilled about a year and a half ago.  The other time is in a ball with maybe 40 games on it.  Some of my oldest and most used equipment is going strong after hundreds of games.   Is anyone else having this issue?  I feel like the thumb is making direct contact with the head pin and just shoving the IT down into the hole and shattering the system?  Every single time it's happened the ball has come back with the slug inside, so it's not breaking upon release.  The slug itself is still totally usable.  Could this be caused by a strange combination of drilling and my PAP making the ball hit the pins around my thumb hole?  I'm just baffled by this.

Have you verified the IT is flush with the surface of the balls in question? I would double check everything. If the slug is not flush I could understand the issue you describe. It could a simple overlook on drill or even pitch that could create the slug not being totally flush.

What does the pro shop operator think the problem is? I don't think they would want any negative feedback.

The pro-shop operator has told me I'm pretty much his only customer that has this issue, and that it has to be a problem with me screwing in the IT too hard or too tight.  I am about 99% sure that this isn't the issue though because it would make sense that my most used balls would break more often than the ones I use less often.  My Arch Rival has over 300 gamers on it, my C System Maxxed Out is over 150 games, and I have a Revolver with over 150 games as well, and all of those are still on the original system. 

The thumb always seems to be flush with the ball, and like I said, up until last year I had never had a single issue with these.  I thought maybe they changed something in their process or made them slightly differently from the factory and for some reason I was one of the few people affected.  Then I realized it always happens to the same ball.  I've tried plugging it entirely and installing a new one, but it still has the same issue from time to time.  So aside from the situation I described above, I'm not really sure what else it could be. 

I really don't want to use another system, the local shop here doesn't have switch grips in stock, I might be able to get him to order them for me, but I wasn't sure if he'd need special drill bits or other tools.
