Brackets - Form?

Does anyone have a link to a standard 8 participant "Bracket Form".  I could make one but feeling a little lazy an thought someone on here might have one made or a link to one.

If you are looking for an inexpensive one, try Braxion.com (I think that is the spelling). It cost me about $50 a few years ago, and is a very simple progrm to use. Very user friendly, can accomodate over 200 brackets at one time. I used it for league (scratch and handicap) as well as in some sweeper tournaments. Very good program.
