What is generally the cause of this? How can it be rectified? I have a friend who does this, and I told him it is a combination of his release characteristics coupled with the ball layout. Is this about right? I threw his ball and, while I tracked very high, I didn't cross the holes. I think he was throwing an AZO Carbide (not too familiar with their equipment) and pin was left of ring with CG in palm. Not a real uncommon layout, but maybe not suited for his game.
