Question about "Green Form" for Nationals

I tried to call the USBC office to find out what to do with the "Green Form" that was mailed to me late last week.  I have been unsuccessful in contacting them, does this form have to me mailed back or is it presented at check-in at the tournament site?  I do have 3 bowlers on the team who are in the Classified Division.

 

I haven't been since 2008, I do not remember the procedure for this form, any help would be greatly appreciated.

I have always presented mine at check-in time.  Its the only time I have my full team together to fill out and sign the form.

Just fill the green form out with all the info. Next, have your teammates sign then present it at the time you check in. I do this every year.
