How do you break out of a slump?

The best thing to do is get yourself on camera in practice and identify what the issue are and then practice them. When you feel you are throwing the ball great, get a few via shots that you can keep in storage as a control, Then compare. It will help when you get into a slump again, (because they happen) you'll have reference to the way you were throwing the ball and the way you're throwing now. Continue to practice and stay positive.

It can't hurt to schedule a few sessions with a good coach.Sometimes they can see small things that you won't pick up on yourself.

Rightycomplex nailed it. Folks never seem to take the time to videotape when they are rolling good (me included).  It saves a lot of time when you have that good baseline to compare it to.

Most of the time a slump starts as a small physical problem but turns into a mental problem. Just focus on the physical game. Try not to get frustrated and logically think your way through it. Dont over analyze every shot and it will all fall back into place.

My suggestion....

Start with the fundamental...
Practice with only 1 ball until you can figure out your form..
When lost - the straighter the better until you can figure out everything else..
